A medical bill is read by more people than almost any other invoice: the patient, a relative paying on their behalf, an insurer's assessor, and sometimes an employer. Each of them needs something different from it, and none of them is in a good mood.
Itemisation is what satisfies all of them at once. A line per item of care answers the assessor, justifies the total to the patient, and leaves you with a document you can defend a year later.
This page is about billing, not clinical practice. Follow your own regulator's rules on what may appear on a patient account and on patient confidentiality.
Line items
What goes on a medical bill
Itemised care, with the payer's portion separated from the patient's.
Provider and registration
Practice or facility name, address, and the practitioner or facility registration number. Insurers and schemes reject bills without it.
Patient and file reference
Name and the patient or file number. Where the bill is going to an insurer, their member and authorisation numbers too.
Date of service
The date care was given, not the date the bill was produced. Claims are assessed against the service date.
Itemised care
Consultation, each procedure, each investigation, each item dispensed — separately priced. A single “treatment” line is the most common reason a claim is queried.
Scheme portion
What the insurer or scheme covers, deducted on its own line with the authorisation reference, leaving the patient balance unambiguous.
Payment and balance
What is due now, how to pay, and whether an instalment arrangement applies.

Where medical bills cause problems
- One line for “treatment”. An assessor cannot approve what they cannot see. Itemise.
- Service date missing. Claims are assessed on the date care was given, not the date of billing.
- Scheme portion netted off silently. Bill the full amount and deduct visibly, or the bill cannot be matched to the claim.
- Clinical detail on a document that travels. Procedure descriptions, yes. Diagnosis narrative that an employer will read, no.
- No registration number. Many schemes reject on this alone, and you will not find out for six weeks.
- Medicines bundled with the procedure. Dispensed items are usually reimbursed under different rules. Separate lines.
Bills at the point of care
Invoice & Quotation Generator produces the same itemised bill on a phone or tablet, which suits a small practice, clinic or pharmacy without a billing system.

Questions
Questions
How do I create a medical invoice or hospital bill?
Fill in the bill above: your practice or hospital name and registration, the patient and any insurer or scheme reference, the date of service, then each consultation, procedure, test and medicine as its own line, followed by tax where it applies and the total.
What should a medical bill include?
Provider name and registration number, patient name and file number, date of service, an itemised list of consultations, procedures, investigations and medicines with individual prices, any insurance or scheme portion deducted, the patient's balance, and how to pay.
Should a medical bill show a diagnosis?
Only what the payer requires, and only with the patient's knowledge. A bill often travels further than a clinical record — to an employer, an insurer, a family member — so put procedure codes and descriptions on it, not clinical detail that is nobody else's business.
How do I show an insurance or scheme payment?
Bill the full amount, then deduct the insurer's portion on its own line with the scheme name and authorisation number, leaving the patient balance clear. Billing only the balance makes the document impossible to reconcile against the claim.
Can this be used for a private clinic or pharmacy?
Yes. The structure is the same for a consultation, a procedure or a dispensed prescription: itemised lines, tax where it applies, a total, and a receipt when payment is made. Use the receipt maker for the payment side.
